Learning objectives

After completing this module you should be able to:

  • Analyze the continuum of potential partnerships across all post acute care sites in your community/market
  • Describe opportunities for improvement in key post acute care providers’ readmission rates and quality metrics for your bundled conditions
  • Facilitate interventions such as:
    • Requiring key provider partners to design interventions aimed at reducing avoidable readmissions
    • Promoting strategic PAC investments in staff training and education, data systems, 90 day cross site care protocols, and prospective care coordination.
